Making the Strategic Shift

The Making the Strategic Shift programme works collaboratively across the NWJIP within the Early Intervention and Prevention programme.

Our key aim is to support authorities in the region to achieve a 'strategic shift' towards prevention by providing information and support to our stakeholders.  

The programme aims to enhance learning and understanding of the best practice available in the five key areas of: reablement, crisis, care pathways, telecare and supported housing. 

The shift towards substantially more early intervention and prevention services within the health and social care environment is a major and integral part of the transformation agenda that is aimed at enabling people to live as independently as possible.

Promoting independence, achieving personalisation, delivering care closer to home and effectively supporting those with long-term conditions in the community all depend on this strategic shift in existing provision and investment within health, housing and social care.

The scale of transformational change produces an equally significant financial impact so as to respond to the dual challenge of a tightening financial situation and increased expectations and awareness of social care needs.

 

Objectives

The overall aim continues to be to provide the right services in the right place, at the right time and we aim to achieve this by continuing the strong collaborative ethos that has enabled agreed best practice to be absorbed from across the country.

Further objectives are to:

  • ensure effective collaboration, knowledge sharing and management of change support across the region
  • provide current regional activity position for each authority in the region and the progress made/current barriers addressed
  • specific support to individual councils wishing to enhance, review or extend their prevention and early intervention services or improve integration at either council or health and social care levels, and
  • make a positive difference to individual outcomes from improved social care prevention and early intervention services
  • strategic planning strengthened and delivery plan actioned.
