The Place Programme
This project is being developed with the aim of promoting a joined up approach across health and local government to meet the needs of frail older people and supports the development of a QIPP initiative across the NHS and local government.
The regional NHS observatory and improvement resource, Advancing Quality Alliance (AQuA), has agreed to adopt the proposal to develop a whole system data and improvement project. The project will involve the development of a locality whole system data scorecard for older people. The data will enable local authorities and the NHS, at a local authority boundary level, to understand the strengths and weaknesses of the local whole system in terms of promoting independence and choice for older people.
